Medical Form & Risk Consent Form: these forms are referenced in both the Agreement for Service and the Programme Request Form, however if we are providing an In-School activity, if you have Blanket Consent for your students participating in the specific EOTC activities outlined within your Programme Proposal, the Risk and Medical Consent are not required. Please indicate on the Programme Request Form if your school’s Blanket Consent covers all activities for the programme.
